I have found, for my clients, that greylisting works extremely well and 
stops about 90% of the spam we were getting, dead in its tracks.  you 
do need to add in Google, Yahoo, Ebay and Amazon netblocks however.

It also follows the nice principle that if someone legitimate does in 
fact have a problem, they get a bounce and then can know that the email 
was not delivered.
